Evaluación in vitro de la actividad antibacteriana de diversas suturas quirúrgicas provistas de clorhexidina frente a triclosán

Introducción: El empleo de suturas quirúrgicas y otros dispositivos implantables con actividad antibacteriana puede representar una estrategia de interés clínico para reducir las infecciones posoperatorias. En el presente estudio se ha valorado el comportamiento de diversas suturas antimicrobianas provistas de los antisépticos clorhexidina (Assufil Plus, Monofil Plus y Assufil Mono Plus) o triclosán (Vicryl Plus, Monocryl Plus y PDS Plus) en un modelo experimental in vitro de infección bacteriana. Métodos: Fragmentos estériles de las distintas suturas (longitud de 1 cm, calibre 2-0), así como de sus respectivos controles (misma sutura, sin antiséptico), fueron inoculados con 106 UFC/mL de Staphylococcus aureus (sensible/resistente a meticilina, S. epidermidis, no formador/formador de biofi lm) y Escherichia coli. Durante un período de 24-72 horas, se evaluó la capacidad de estos materiales para desarrollar zonas de inhibición (test de difusión en agar), alterar el crecimiento bacteriano en cultivo (turbidez) e impedir la adhesión bacteriana a su superfi cie (sonicación, microscopía electrónica de barrido). Resultados: Mientras que las suturas control se encontraron fuertemente colonizadas, todas las suturas provistas de clorhexidina o de triclosán desarrollaron halos de inhibición estables sobre la superficie del agar. De forma similar, la presencia de estos materiales en el cultivo evitó el crecimiento bacteriano, así como la adhesión de los microorganismos a su superficie. Conclusión: El comportamiento de las suturas antimicrobianas provistas de clorhexidina o triclosán es adecuado en condiciones experimentales in vitro. Con ambos antisépticos se evita la adhesión bacteriana al material de sutura.
